AIMS: The aim of this study is to report perceived discrimination among Muslims living in Norway and to address and compare associations between perceived discrimination and health among Muslims with an immigrant background and other-religious with an immigrant background. METHOD: A representative sample of individuals with an immigrant background in Norway was used in a cross-sectional study design that included 5484 respondents aged 16 to 74 years. The respondents were sub-grouped after religious affiliation, and as immigrants and Norwegian-born. This sample is from 'The Survey on living conditions among persons with an immigrant background 2016', conducted by Statistics Norway. Multivariate logistic regression analyses were conducted to investigate the relationship between perceived discrimination and self-rated health and between perceived discrimination and mental health problems. RESULTS: Our findings show that Muslims with an immigrant background are more likely to report perceived discrimination than non-Muslims with an immigrant background. Perceived discrimination was associated with poor self-rated health and mental health problems among immigrant Muslims and Norwegian-born Muslims. Among other-religious with an immigrant background, perceived discrimination had an inverse relationship with mental health problems among immigrants, while an association between perceived discrimination and poor self-rated health was found among Norwegian-born. CONCLUSIONS: Our findings suggest that perceived discrimination does play a role in health among minorities with an immigrant background in Norway, regardless of religion. However, the association between perceived discrimination and poor health seems to be stronger among Muslims, especially Norwegian-born Muslims.

BACKGROUND: Infectious diseases remain a major risk for refugees living in camps. Limited research has been undertaken into the burden and impact of infections in the current refugee crisis around the Mediterranean.This analysis examines the frequency and incidence rates of common infections, and use of antibiotics at a healthcare facility inside Moria refugee camp on the island of Lesvos, Greece, during the winter and summer seasons of 2019. METHODS: This is a retrospective analysis of routinely, prospectively collected service data from the main acute healthcare facility inside Moria camp. RESULTS: Of a total of 9601 consultations, 25.16% were for infections (winter: 31.53%, summer: 19.45%). Respiratory, skin and gastrointestinal infections were the most frequent, with incidence rates up to 3.5 times, 50 times and twice as high as those in Western Europe, respectively. Antibiotic prescribing was high (27.7% of all respiratory infections) with high use of broad-spectrum antibiotics (40.2%), raising concerns about induction of antimicrobial resistance. CONCLUSIONS: The burden of infectious diseases in refugee camps remains high throughout all seasons. Antimicrobial stewardship programmes should be adapted to refugee camp settings to prevent the development of unnecessary antimicrobial resistance. Improving living conditions and access to healthcare in refugee camps is likely to reduce infection rates and antimicrobial resistance among refugees.

PURPOSE: Adolescent emergency department (ED) patients have unmet social needs that contribute to ED use. This study aimed to evaluate the effect of social needs navigation for adolescents on subsequent ED visits and community resource use and to identify characteristics associated with elevated social risk. METHODS: Between July 2017 and August 2019, we used a random date generator to establish intervention and control group enrollment dates. All adolescents completed a social needs survey. Adolescents enrolled on intervention dates received in-person, risk-tailored social needs navigation. Those enrolled on control dates received a preprinted resource guide. We used chart review and follow-up calls to assess 12-month ED revisits and community resource use. Logistic regression was used to compare these outcomes between groups. We measured the association between ≥3 reported unmet needs and characteristics hypothesized a priori to be associated with elevated social risk (nonurgent visits, obesity, or any of nine "socially sensitive" chief complaints) using logistic regression. RESULTS: A total of 399 adolescents were randomized. There was no difference between groups in the number of ED revisits. There was increased community resource use in the intervention group (adjusted odds ratio [aOR]: 3.5 [95% confidence interval {CI}: 1.5, 8.2]). Adolescents with a socially sensitive chief complaint had increased odds of ≥3 unmet needs (aOR: 2.2 [95% CI: 1.3, 3.6]), as did those with food insecurity in a post hoc analysis (aOR: 9.9 [95% CI: 4.0, 24.6]). DISCUSSION: Social needs navigation increased community resource use but not subsequent ED visits. Adolescents with socially sensitive chief complaints or food insecurity reported increased unmet needs.

RESUMO

Health-related social needs (HRSNs), such as food insecurity and housing instability, drive health and well-being. The socioeconomic impacts of the COVID-19 pandemic increased the prevalence of HRSNs and highlighted the critical need for strategies to address those needs, particularly in communities experiencing health disparities. Implementing HRSN screening requires adopting effective strategies to overcome common challenges. This report synthesizes promising implementation approaches and lessons learned from the Accountable Health Communities Model, a national effort funded by the Centers for Medicare & Medicaid Services Innovation Center to systematically screen for and address HRSNs in clinical settings. Key strategies include increasing patient engagement and building trust through culturally tailored language and outreach; using and sharing data for monitoring and improvement; using technology to expand access to screening and referrals; dedicating staff to screening roles; integrating screening into existing workflows; and building buy-in among staff by communicating the impact of screening and encouraging peer connections.

AIM: To explore how overcrowding affects children's health, environment and schooling. METHODS: A qualitative study was conducted with individual interviews among 20 participants with occupational experience from overcrowded Stockholm areas but diverse in professions, locations and employers. The interviews were recorded, transcribed and analysed with Systematic Text Condensation. RESULTS: Almost all participants expressed that overcrowding has a negative impact on children's health, environment and schooling - based on perceptions of precarious and different living conditions for children in overcrowded areas, for example, substandard homes, vulnerability, stress, exclusion, limited resources, lack of learning opportunities, gender differences, confinement, shame, insecurity, conflicts, risk of criminality, and bodily impact, both physical and psychological. CONCLUSIONS: Our qualitative evidence suggest that overcrowding has a negative impact on children's health, environment and schooling.

Objetivo: Evaluar el fenómeno de violencia sexual y condición de vida de mujeres afrocolombianas víctimas del conflicto armado, asentadas en la ciudad de Cartagena, Colombia. Materiales y métodos: Estudio prospectivo de corte transversal, tipo encuesta poblacional, realizado entre septiembre de 2019 y marzo del 2020, en la ciudad de Cartagena, Colombia. Se evaluaron variables sociodemográficas, tipo de delitos sufridos en el conflicto armado y factores relacionados, además de antecedentes o presencia de sintomatología psiquiátrica asociada a los eventos traumáticos. A través de análisis descriptivo, se caracterizó las condiciones de vida actuales de las participantes y se exploraron asociaciones entre la condición de violencia sexual y sintomatología psiquiátrica, por medio de Odds Ratio (OR). Resultados: Participaron 215 mujeres, con una mediana de edad de 40 años y desplazadas principalmente del departamento de Bolívar (55%). El 85% eran amas de casa y solo el 60% tiene vivienda propia, encontrándose más del 65% del total de las viviendas en zonas de riesgo por inundación. Por lo menos el 35% habían sufrido discriminación de género, racial o habían sido víctima de violencia sexual. Aproximadamente la mitad de la muestra declaró padecer insomnio, ansiedad y depresión. El 40% mencionó haber tenido pensamientos suicidas y solo 1 de cada 5 mujeres recibió apoyo psicológico. Haber sido secuestrada (OR 4,71; IC 95%, 1,05-21,07), perseguida (OR 3,33; IC 95%, 1,61-6,89) y abusada sexualmente (OR 3,09; IC 95%, 1,60-5,96), son hechos que se asociaron de forma sostenida y significativa con sintomatología psiquiátrica. Conclusión: Las mujeres afrocolombianas víctimas del conflicto armado colombiano asentadas en Cartagena, presentan condiciones de vida inestables, no han recibido la ayuda adecuada de redes de apoyo, han sido discriminadas por su etnia, género y condición de víctima, y tienen una prevalencia elevada de sintomatología psiquiátrica asociada a los hechos traumáticos vividos.


Objective: To evaluate the phenomenon of sexual violence and the living conditions of Afro-Colombian women victims of the armed conflict, who lives in city of Cartagena, Colombia. Materials and methods: Prospective cross-sectional study, population-based survey type, conducted between September 2019 and March 2020, in the city of Cartagena, Colombia. Sociodemographic variables, type of crimes suffered in the armed conflict and related factors were evaluated, as well as history or presence of psychiatric symptoms associated with traumatic events. Through descriptive analysis, it was characterized the current living conditions of the participants; and also explored associations between the condition of sexual violence and psychiatric symptomatology, through Odds Ratio (OR). Results: 215 women participated, with a median age of 40 years and displaced mainly from the department of Bolívar (55%). 85% percent were housewives and only 60% have their own house, with more than 65% of the total number of houses located in flood risk areas. 35% had suffered gender or racial discrimination or had been victims of sexual violence. Approximately half of the sample reported suffering from insomnia, anxiety and depression. 40% mentioned having had suicidal thoughts and only 1 in 5 women received psychological support. Having been kidnapped (OR 4.71; 95% CI, 1.05-21.07), persecuted (OR 3.33; 95% CI, 1.61-6.89) and sexually abused (OR 3.09; 95% CI, 1.60-5.96), are events that were consistently and significantly associated with psychiatric symptomatology. Conclusion: The Afro-Colombian women victims of the Colombian armed conflict who lives in Cartagena, present unstable living conditions, have not received adequate help from support networks, have been discriminated against because of their ethnicity, gender and victim status, and have a high prevalence of psychiatric symptoms associated with the traumatic events experienced.

OBJECTIVES: Although the association between adverse childhood conditions and health in later life has been relatively well established, little is known about how and through which mechanism this association develops. Building on the developmental adaptation model (Martin & Martin, 2002), the present study investigates the effects of distal and proximal influences on successful aging (SA). METHODS: A cross-sectional survey was conducted of 475 adults aged 50 and above (Mean age = 72.13, SD = 10.46). SA was measured based on Rowe and Kahn's successful aging criteria model (1997) and an SA operationalization based on a number of multidimensional biopsychosocial indicators, including no disease and disability, active engagement with life, high physical and cognitive functioning, psychological well-being, life satisfaction and a one-item subjective SA question. RESULTS: Parental death had no direct effect on SA (ß = .03, p = .629), whereas education had (ß = .39, p < .001). Furthermore, perceived income level and social conditions were identified as two sequential mediators between both education and SA; and parental death and SA. CONCLUSIONS: The findings suggest that both early life and current factors contribute to explaining SA in a variety of ways.

PURPOSE: Studying breast and cervical cancers in space and time and verifying divergences of different territorially established socioeconomic profiles. METHODS: Ecological study using spatial scanning (with socioeconomic characterization), space-time, and spatial variation of temporal trends, in order to identify significant clusters of high- and low-risk or temporal trends, of deaths from breast cancer and cervical cancer, in the city of São Paulo, Brazil, during 2000-2016. RESULTS: High-risk spatial clusters were identified in the central areas, and low-risk clusters were identified in the peripheral areas, which were associated with better and worse socioeconomic conditions, respectively. As for cervical cancer, the pattern was the opposite. High-risk space-time clusters occurred in the early years of the study, whereas low-risk clusters occurred in the most recent years. For breast cancer, the central areas showed a temporal trend of decreasing mortality and the peripheral areas showed an increasing trend. While for cervical cancer, in general, the temporal trend was for the identified clusters to fall. CONCLUSIONS: It is expected that this study will provide insights for the formulation of public policies to implement prevention and control measures, in order to reduce mortality and inequalities related to breast and cervical cancers.

RESUMO Objetivo Identificar os fatores relacionados ao aleitamento materno exclusivo até o sexto mês de vida dos bebês em serviços de Atenção Primária à Saúde. Método Estudo quantitativo, transversal, realizado com 261 mães com bebês de 12 meses, procedentes de unidades de saúde de Porto Alegre. Dados relativos ao aleitamento materno exclusivo, características sociodemográficas, saúde mental materna, relações familiares e acompanhamento de puericultura foram analisados através de testes t, qui-quadrado e modelo regressão de Poisson. Resultados A taxa de aleitamento materno exclusivo até o sexto mês de vida foi de 41%. A mãe ter companheiro aumentou em 46% os índices de aleitamento exclusivo, enquanto retornar ao trabalho antes dos seis meses reduziu em 31% as chances de amamentação exclusiva. Não foi identificada associação significativa entre aleitamento materno exclusivo e variáveis sociodemográficas, saúde mental materna, relações familiares, e acompanhamento de saúde da criança. Conclusão A presença de companheiro e o retorno da mulher ao trabalho após os seis meses de vida do bebê foram identificados como protetores ao aleitamento materno exclusivo. Intervenções em saúde que fomentem a rede de apoio, políticas de incentivo à prática e transmissão de conhecimentos sobre amamentação são importantes para melhores índices de aleitamento materno exclusivo.


ABSTRACT Purpose To identify factors related to exclusive breastfeeding up to the sixth month of life of babies in Primary Health Care services. Methods Quantitative, cross-sectional study, carried out with 261 mothers with 12-month-old babies, coming from health units in Porto Alegre. Data related to exclusive breastfeeding, sociodemographic characteristics, maternal mental health, family relationships and childcare follow-up were analyzed using t-tests, chi-square and Poisson regression model. Results The rate of exclusive breastfeeding until the sixth month of life was 41%. The mother having a partner increased the rates of exclusive breastfeeding by 46%, while returning to work before six months reduced the chances of exclusive breastfeeding by 31%. No significant association was identified between exclusive breastfeeding and sociodemographic variables, maternal mental health, family relationships, and child health monitoring. Conclusion The presence of a maternal partner and the woman's return to work after the baby's six months of life were identified as protectors to exclusive breastfeeding. Health interventions that promote the support network, policies to encourage the practice and transmission of knowledge about breastfeeding are important for better rates of exclusive breastfeeding.

Objetivo: analisar as condições de saúde da população atingida pelo rompimento da barragem de mineração na perspectiva dos profissionais de saúde e gestores. Método: estudo de caso qualitativo, realizado por meio de documentos de domínio público e entrevistas com profissionais de saúde e gestores, entre os meses de março a maio de 2022, totalizando 14 participantes. Os dados foram analisados segundo a estratégia de proposições teóricas à luz da Determinação Social da Saúde. Resultados: considera-se como piora nas condições de saúde em relação aos problemas respiratórios, gastroenterites, obesidade, arboviroses, intensificação do uso de álcool e drogas, e doenças mentais. Destaca-se a perda do trabalho, da casa, e do pertencimento comunitário dos atingidos. Conclusão: o desastre socioambiental foi condicionante para mudanças nas condições de saúde dos atingidos, expondo-os a novos cenários e sobreposição de riscos capazes de modificar a qualidade de vida e agravar as condições de saúde biopsicossociais.


Objective: to analyze the health conditions of the population affected by the mining dam collapse from the perspective of healthcare professionals and managers. Method: this is a qualitative case study, conducted using public domain documents and interviews with healthcare professionals and managers, from March to May 2022, totaling 14 participants. The data were analyzed according to the theoretical propositions strategy based on the Social Determinants of Health. Results: the health conditions of this population have worsened in terms of respiratory problems, gastroenteritis, obesity, arboviruses, increased use of alcohol and drugs, and mental illnesses. The loss of employment, home, and community belonging of those affected is highlighted. Conclusion: the socio-environmental disaster was a conditioning factor for changes in the health conditions of those affected, exposing them to new scenarios and overlapping risks capable of modifying their quality of life and aggravating their biopsychosocial health conditions.


Objetivo: analizar las condiciones de salud de la población afectada por el colapso de la presa minera desde la perspectiva de los profesionales y gestores sanitarios. Método: estudio de caso cualitativo, realizado por medio de documentos de dominio público y entrevistas a profesionales y gestores de salud, entre marzo y mayo de 2022, con un total de 14 participantes. Los datos fueron analizados según la estrategia de proposiciones teóricas a la luz de la Determinación Social de la Salud. Resultados: se considera el empeoramiento de las condiciones de salud en relación con los problemas respiratorios, la gastroenteritis, la obesidad, los arbovirus, la intensificación del consumo de alcohol y drogas, y las enfermedades mentales. Se destacan la pérdida del trabajo, del hogar, y de la pertenencia a la comunidad de los afectados. Conclusión: el desastre socioambiental fue un factor condicionante de alteraciones en las condiciones de salud de los afectados, exponiéndolos a nuevos escenarios y riesgos superpuestos capaces de modificar la calidad de vida y empeorar las condiciones de salud biopsicosocial.

Este artigo aborda as experiências das catadoras de materiais recicláveis diante das implicações provocadas pela pandemia da COVID-19 em uma associação no Distrito Federal (DF). O objetivo principal foi compreender a situação de vida precária das mulheres catadoras inseridas em uma desigualdade sistêmica e institucional do capitalismo. Para tanto, foi realizada uma etnografia que combinou técnicas de observação, interação e escuta (conversas informais e entrevistas semiestruturadas). Através disso, permitiu-se o acompanhamento do cotidiano de vida dessas mulheres, desde a instauração de uma das maiores crises sanitária da humanidade até o período do retorno às atividades laborais. Os resultados revelaram que as catadoras estão inseridas na lógica de precarização da vida sendo que as condições foram agravadas com a chegada do novo coronavírus quando aumentou a vulnerabilidade relacionada ao vínculo de trabalho informal, a insalubridade e os riscos à saúde inerentes à ocupação e às dificuldades de acesso aos serviços de saúde e assistência social. O contexto mostra uma lida complexa relacionada ao medo do desemprego, às dificuldades e às barreiras do ingresso no mercado de trabalho e de manutenção da própria subsistência. Sobretudo, isso agravou-se de forma mais intensa diante da ausência de políticas e programas voltados para elas: o Estado fez pouco para a redução dos impactos da pandemia em suas vidas. Assim, a crise sanitária instaurada refletiu a fragilidade delas em face ao ciclo da reciclagem. Este fato demonstra a necessidade urgente de ações governamentais que abarquem esse grupo social tanto no momento da pandemia quanto pós-pandemia da COVID-19, visando superar a invisibilidade social


This article addresses the experiences of women waste pickers in the face of the implications caused by the COVID-19 pandemic in an association in the Distrito Federal (DF). The main objective was to understand the precarious life situation of women waste pickers inserted in a systemic and institutional inequality of capitalism. For that, an ethnography was carried out that combined observation and listening techniques (informal conversations and semi-structured interviews). Through this, it was possible to monitor the daily life of these women, from the onset of one of the greatest health crises in humanity until the period of return to work activities. The results revealed that the waste pickers are inserted in the logic of precariousness of life, and the conditions were aggravated with the arrival of the new coronavirus when the vulnerability related to the informal work bond, the unhealthy conditions and health risks inherent to the occupation and the difficulties increased. access to health and social assistance services. The context shows a complex deal related to the fear of unemployment, the difficulties and barriers to entering the labor market and maintaining one's livelihood. Above all, it worsened more intensely in the face of the absence of policies and programs aimed at them, the State did little to reduce the impacts of the pandemic on their lives. Thus, the established health crisis reflected their fragility in the face of the recycling cycle. This fact demonstrates the urgent need for government actions to embrace this social group, both during the pandemic and post-pandemic of COVID-19, aiming to overcome social invisibility

Resumen Se propone desarrollar la articulación entre los aportes teórico-metodológicos de la terapia ocupacional social y la experiencia de los proyectos Timbó em Movimento y ResisTO, dentro del núcleo de la Rede Metuia perteneciente a la Universidad Federal de Paraíba, Brasil. Se reconoce la importancia de las prácticas territoriales y del campo de la terapia ocupacional social para el fortalecimiento y acompañamiento a poblaciones con diversas condiciones de vulnerabilidad. Se articula lo observado en los proyectos Timbó em Movimento y ResisTO con conceptos como la interseccionalidad de los marcadores sociales de diferencia, sus implicancias en el derecho a la ciudad y sus efectos en el cotidiano de los jóvenes. Se desarrolla la implicancia de los marcadores sociales en los cotidianos de les participantes, así como el potencial de los espacios colectivos de referencia y la articulación intersectorial para enfrentar las violencias. Se concluye que los espacios de pertenencia poseen gran relevancia para el abordaje de conflictos en el campo social, potenciando la participación de les sujetes y el ejercicio de ciudadanía, rechazando la tendencia de buscar soluciones individuales a problemáticas colectivas. Se identifica el desafío de llevar estos diálogos a los contextos cotidianos en que las violencias tienen lugar, incorporando actores que también puedan estar involucrados en su reproducción.

Identifica-se o desafio de levar esses diálogos aos contextos cotidianos em que as violências ocorrem, incorporando atores que também podem estar envolvidos em sua reprodução.


Abstract This paper articulates the theoretical-methodological contributions of social occupational therapy and the experience of "Timbó em Movimento'' and "ResisTO" projects, which are part of the Rede Metuia nucleus belonging to the Federal University of Paraíba (UFPB), Brazil. It recognizes the importance of territorial practices and the social occupational therapy field for the strengthening and support of populations with diverse conditions of vulnerability. Articulation of what was observed in the Timbó em Movimento and ResisTO projects with concepts such as the intersectionality of social markers of difference, their implications for the right to the city and their effects in the daily life of youths. The article elaborates the implications of social markers in the daily life of the participants, as well as the potential of collective reference spaces and intersectoral articulation to confront violence. It is concluded that the spaces of belonging have great relevance for addressing conflicts in the social field, promoting the participation of people and the exercise of citizenship, rejecting the tendency to seek individual solutions to collective problems. Remains the challenge of taking these dialogues to the daily contexts in which violence takes place, incorporating actors that may also be involved in its reproduction.

Objetivo: Conhecer a percepção de pessoas em situação de rua em relação ao tratamento da tuberculose. Métodos: Estudo qualitativo, com base na história oral temática. Foram entrevistadas 24 pessoas em situação de rua no município de São Paulo, em tratamento da tuberculose e vinculados a duas Unidades Básicas de Saúde, no período de junho a julho de 2018. Resultados: Emergiram três categorias: A situação de rua como desafio para o enfrentamento da tuberculose; o impacto do tratamento na vida dos usuários; as potencialidades e os limites dos serviços de saúde no apoio ao tratamento. Os participantes destacaram que o fato de viver em situação de rua impacta no tratamento da tuberculose, pois as medidas de isolamento e monitoramento são menos efetivas. Em contrapartida, a possibilidade de cura e a relação estabelecida com os profissionais de saúde foram fundamentais para a adesão ao tratamento. Considerações finais: A tuberculose na população de rua deve ser compreendida na perspectiva da determinação social do processo saúde-adoecimento. Por outra parte, desafios inerentes aos esquemas terapêuticos, condições de vida na rua, vulnerabilidades individuais, coletivas e programáticas dos serviços e políticas públicas que precisam ser considerados na construção das estratégias de cuidado. Objective: To understand homeless people's perception in relation to the treatment of tuberculosis. Methods: A qualitative study based on thematic oral history. Twenty-four homeless individuals from the city of São Paulo undergoing treatment for tuberculosis and linked to two Basic Health Units were interviewed from June to July 2018. Results: Three catego-ries emerged: Homelessness as a challenge to coping with tuberculosis; The impact of treatment on users' lives; and The potential and limits of health services in supporting the tuberculosis treatment. The participants highlighted that the fact of living on the streets exerts impacts on their treatment, as isolation and monitoring measures are less effective. On the other hand, the possibility of cure and the relationship established with health professionals were fundamental for adherence to the treatment. Final considerations: Tuberculosis in the street population must be understood from the perspective of the social determination of the health-disease process. Added to the challenges inherent to the treat-ment of tuberculosis, life conditions on the street, as well as individual, collective and programmatic vulnerabilities of the services and public policies need to be considered in the care strategies. Introducción: Los primeros años de la vida son dinámicos en comparación con todo el proceso de crecimiento, el cual se caracteriza por su vulnerabilidad ante las condiciones sociales, económicas y ambientales, lo que hace necesario un estado nutricional adecuado. Objetivo: analizar los factores asociados a la desnutrición aguda en niños menores de cinco años en el Valle del Cauca entre 2016 y 2019. Metodología: se realizó un estudio descriptivo de corte transversal con análisis correlacional a la luz de los determinantes sociales en salud. La población estuvo conformada por 2.237 niños/as registrados en la base de datos básicos y complementarios de la ficha epidemiológica del evento 113 del Sistema de Vigilancia en Salud Pública. Se concluyó que los más afectados por desnutrición fueron los niños de sexo masculino con un 55,4% y en edades entre los seis y 12 meses (62%). Resultados: entre la información relativa al peso, el 78,6% tuvo un normopeso al nacer, el 88% estaban inscritos en el programa Crecimiento y Desarrollo y la mayoría tenían residencia en el área de la cabecera municipal. Las madres o cuidadores que alcanzaban un grado de escolaridad de secundaria eran un 56,73%. Conclusiones: En esa medida, en el Valle del Cauca la presencia de desnutrición aguda se relaciona con el tiempo de lactancia materna, la inscripción al programa de Crecimiento y Desarrollo, la edad del menor, la pertenencia étnica, el régimen de salud, el nivel educativo de la madre y el área de residencia.(AU)


The first years of life are dynamic compared to the entiregrowth process, which is characterized by its vulnerability tosocial, economic and environmental conditions, which makesit necessary to have an adequate nutritional status. The ob-jective of the present research consisted of analyzing the fac-tors associated with acute malnutrition in children under fiveyears of age in Valle del Cauca between 2016 and 2019. A de-scriptive cross-sectional study was conducted with correla-tional analysis in light of the social determinants of health.The population consisted of 2,237 children registered in thebasic and complementary database of the epidemiologicalrecord of event 113 of the Public Health Surveillance. The in-formation related to weight, 78,6% had normal weight atbirth, 88% were enrolled in the Growth and DevelopmentProgram and most of them lived in the area of the municipal capital. The mothers or caregivers had 56,73% secondaryschooling. Thus, in Valle del Cauca, the presence of acutemalnutrition is related to the length of breastfeeding, enroll-ment in the Growth and Development Program, age of thechild, ethnicity, health regime, educational level of the motherand area of residence. It was concluded that the most af-fected by malnutrition were males (55.4%) and aged be-tween six and 12 months (62%).(AU)

Sleep is a relevant factor for functioning and well-being of young people. The paper provides a differentiated description of sleep difficulties in this population group including social, health-related, and environmental factors. The analyses included n=6,728 11- to 17-year-olds of the KiGGS baseline study (2003-2006) and 6,072 young adults (age 18-31), who provided information relating sleep in the survey KiGGS Wave 2 (2014-2017). Information from 3,567 people was evaluated at two survey points. 22.0% of the 11- to 17-year-olds reported sleep difficulties. A significant impact for the sex (female), living with a single parent, and with siblings is reflected in the logistic regression. The risk for sleep difficulties increases significantly in the case of mental problems and pain. Among the 18- to 31-year-olds, 19.6% complained of difficulties falling asleep and sleeping through the night. In addition to sex, noise exposure, a low level of education, the professional situation, and living with children were reflected as important influencing factors in the logistic regressions. Over one third of those, who suffered from sleep problems as children and adolescents, also indicated sleep difficulties almost ten years later. The high prevalence of sleep problems and the associated health risks illustrate the high public health relevance of the topic. In addition to sex, health-related and environmental variables also turned out to be significant and need to be considered in the development of interventions.

Background: Despite a tax-funded, needs-based organisation of the Danish health system, social inequality in cancer rehabilitation and palliative care (PC) has been noted repeatedly. Little is known about how best to improve access and participation in cancer rehabilitation and PC for socio-economically disadvantaged and socially vulnerable patients. Aim: To gather, synthesise and describe practice-orientated development studies presented in Danish-language publications and examine the underpinning conceptualisations of social inequality and vulnerability; explore related views of stakeholders working in the field. Methods: The study comprised a narrative review of Danish-language literature on practice-orientated development studies which address social inequality and vulnerability in cancer rehabilitation and PC and an online stakeholder consultation workshop with Danish professionals and academics working in the field. Results: Two themes characterise the included publications (n = 8): types of interventions; conceptualisations of social inequality and vulnerability; three themes were identified in the workshop data: focus and type of interventions; organisation of cancer care; and vulnerability of the healthcare system. The publications and the workshop participants (n = 12) favoured approaches which provide additional individualised resources throughout the cancer trajectory for this patient group. The terms social inequality and social vulnerability are largely used interchangeably, and associated with low income and no or little education yet qualified with multiple descriptors, which reflect the diverse socio-economic situations professionals encounter in cancer patients and their psychosocial needs. Conclusion: Addressing social inequality and vulnerability in cancer rehabilitation and PC in Denmark entails practical and conceptual challenges. Of importance is individualised support and the integration of rehabilitation and PC into standardised care pathways. To conceive of social vulnerability as a layered, dynamic, relational and contextual concept reflects current practice in identifying the diversity of cancer patients who may benefit from additional support in accessing and participating in rehabilitation and PC.

Congenital anomalies (CA) contribute to disabilities and health conditions throughout life. Furthermore, they can cause emotional distress to the mothers and children, who may also experience limitations in individual and social development. This study investigated the prevalence of CA and the relationship with maternal education and age according to local development in the extreme south of Brazil. This is a retrospective observational study with birth data from the Live Birth Information System from 2000 to 2017. The association between age and maternal education with the presence of CA was verified using multiple Poisson regression for robust variances in models adjusted for those variables with a preliminary significant association. A total of 5131 (1.5%) had some CA identified at birth between 2000 and 2017. Only advanced age (≥36 years) was associated with CA regardless of macro-region development (p ≤ 0.001). The highest risk was observed in regions with medium development (RR = 1.60; 95% CI 1.30−1.97). Maternal education (<8 years of study) was associated with CA only in mothers from macro-regions with very high development (RR = 1.27; 95% CI 1.03−1.54). These analyses confirmed that women of advanced age are at greater risk of having children with a CA regardless of maternal education and local development, but social characteristics can also have an influence, as regions with higher development had lower prevalence of CA.

Besides individual determinants, obesity is also caused by obesogenic environmental conditions. Thus, prevention of obesity in children should aim at creating health-promoting environments. This is especially important for children of socially disadvantaged families and communities who are hardly reached and often profit less by behavior-oriented interventions. The main question is which environmental conditions should be changed and how in order to be accepted and sustainable? According to our experiences from the project "Grünau moves" (Grünau bewegt sich), the consideration of environmental and social conditions in the intervention field as well as the inclusion of the professional expertise of local agents and the lifeworld perspective (subjective meanings and realities) of families and children are crucial. Based on the fact that the understanding and weighting of health varies between individuals, the starting points for interventions and environmental change must be sought according to the interests and resources of the local community. Combining a medical and social work perspective and focusing on the community work approach can help to link and bundle different perspectives and interests in order to create appropriate and context-specific health-promoting environments.
